PERMISSION AND RELEASE:

I _______________________(print name), in participating in the 2009 New York City Public Ultimate League (NYCPUL), Discabilities, Ultimate Peace - Annual Spirit of the Game ultimate tourney, understand that danger exists with respect to Ultimate frisbee (the “Activities”) due to many factors, including but not limited to the actions or inactions of others participating, the weather, field, facility, observers or other prevalent conditions. I also understand the nature of the Activities, and acknowledge, represent, and warrant that I am qualified and physically able to fully participate in such Activities and am participating at my own risk.

I release and discharge the NYCPUL, Ultimate Peace and Discabilities, its administrators, organizers or volunteers from any and all claims, actions or liability arising out of or in connection with my participation in the League. This release includes any loss, damage, or liability, or other fault that may result in personal injury, death, or property damage during or in connection with the above League or .Activities This release will be construed according to the law of New York State. This Permission and Release shall inure to the benefit of licensees and assigns of the New York City Public Ultimate League, Ultimate Peace, Discabilities, and shall be binding upon the participant, , his/her heirs, estate, personal representatives and assigns.
